Metal Prince of Darkness Ozzy Osbourne died of cardiac arrest and coronary artery disease, his death certificate reveals.

Ozzy Osbourne’s death certificate, filed at a registry in London and submitted by Osbourne’s second-eldest daughter Aimee, reveals the star’s cause of death. According to the document, the heavy metal legend died of a heart attack and coronary artery disease, as well as complications related to Parkinson’s disease.

Specifically, the document says he died of “(a) Out of hospital cardiac arrest, (b) Acute myocardial infarction, (c) Coronary artery disease and Parkinson’s disease with autonomic dysfunction (Joint Causes).” The document was obtained by The New York Times on Tuesday.

Ozzy Osbourne passed away on July 22, and an air ambulance flew to his home near the village of Chalfont St. Giles in Buckinghamshire, England. A spokesperson for the Thames Valley Air Ambulance service said the team was “dispatched to provide advanced critical care at an incident near Chalfront St. Giles on July 22.”

The ambulance stopped near the singer’s home and then flew about eight miles southwest to Harefield Hospital in the London suburb of Uxbridge, according to data from Flightradar24. The helicopter spent “about an hour” at the hospital with its engines still running.

Ozzy’s family did not reveal a cause of death when they announced his passing on July 22, but the 76-year-old had received treatment for numerous health issues over the past few years. This includes a variant of Parkinson’s disease and a spinal injury, which required surgery. In February, he revealed that he could no longer walk.

At the “Back to the Beginning” concert on July 5, announced back in February by Osbourne’s wife Sharon to be his final performance, Ozzy performed two brief sets. One was of his solo music, while another reunited his Black Sabbath bandmates. That show generated $200 million in charitable donations to benefit a variety of Ozzy’s chosen charities—including one for Parkinson’s research.

“I’ve been laid up for six years,” Ozzy told his fans at the show. “Thank you from the bottom of my heart.”
